There are SIX spaces to write in #’s … I wonder if that means we can submit TWO $15 REBATES???? Why else would there be that many?
I canot print the $ 15.oo rebate form.

Sheila – the Olay rebate form is on the Olay website – not mine. Therefore, I’m not sure how to help with your printing issue. One thing to check is that you have Adobe installed on your computer because the rebate form is a PDF file.
